Open Records Request

Requests to view or receive copies of MSD records subject to the Kentucky Open Records Act must be made in writing. Requests may be made online using the link below, by fax to (502) 540-6109, through United States Postal Service mail or in person at the MSD principal office. The MSD Open Records Policy and fee schedule complies with KRS 61.876 (1) of the Kentucky Open Records Act.

The Director of the MSD Records & Information Governance Program is designated as the official custodian of the agency's public records. The address of the official custodian of public records is that of the principal office of MSD.

Requests must include

  1. Full name of person making the request
  2. Mailing address of person making the request
  3. A description of the record(s) being sought*
  4. Preferred method of contact

*The request should contain as much specific information as possible in order to allow MSD to accurately identify any responsive record(s) in a timely manner.
